Please be aware that the following policies are in effect as of August 1, 2018.


Cancellations/No Show

If an appointment is not cancelled at least 24 hours in advance or you fail to show up to your appointment, then you will be charged a one hundred fifty dollar ($150) cancellation fee. This will not be written on claim forms for insurance reimbursement.

We understand that there are times when you must miss an appointment due to emergencies or obligations; however, we do ask that you call to cancel an appointment in order to allow other patients to receive treatment.


Lateness

If a patient arrives 10 minutes past their scheduled time, then we may have to reschedule their appointment.

We understand that delays can happen. However, we must try to keep other patients, the doctor, and the nurse practitioner on time.


ACCOUNT BALANCES

We require all patients with any balances on their account to pay their account balances to zero dollars ($0) prior to receiving further services from our practice.

Patients who have questions about their bills may speak with any one of our administrative staff members. We are more than happy to review the account and address any questions or concerns.
